Tell us about your condition (5 minutes)

You complete a structured health intake covering your diagnosed condition(s), current treatment, exercise history, and any medications. We turn your answers into a clean clinical summary — written in the language your doctor expects.

We prepare your packet

Within an hour of submission, our compliance team prepares and reviews three documents: a one-page clinical summary, a draft Letter of Medical Necessity formatted for your physician's review, and a patient portal message you can copy-paste into your doctor's office portal.

Send to your own doctor

You log into your existing patient portal (MyChart, Athena, FollowMyHealth, etc.) and send the message with the attached draft letter. Your doctor reviews — typically takes them under a minute — and signs.

Submit for reimbursement

Once you have the signed letter, you pay for your gym, training, or equipment as usual, save the receipts, then submit them with the letter to your FSA/HSA administrator. Most reimbursements process in 7–14 days.

  • Letter is valid for 12 months — renewable annually

What we don't do

Important context for what DoctorNoted is and isn't.

  • We don't diagnose conditions. Your physician retains all clinical authority. We only document conditions they've already diagnosed.
  • We don't sign the letter. Your own primary care physician does. We just prepare the draft and make their part effortless.
  • We don't determine medical necessity. Whether exercise is medically necessary for your condition is between you and your doctor.
  • We don't guarantee reimbursement. Approval is up to your FSA/HSA administrator. We design every part of the packet to maximize approval, but final say isn't ours.
